Terms and Conditions

As part of our commitment to helping you find the right support, Choice Community Health is offering a limited-time deal for new clients engaging with our featured support workers.

By participating in this promotion, you agree to the following Terms and Conditions:

1. Eligibility

   • This offer is valid for new clients only. A “new client” is defined as someone who has never received services from Choice Community Health previously under their NDIS plan.

   • The offer applies exclusively to bookings made through our Facebook campaign featuring individual support worker profiles.

2. Offer Details

   • If, after your first appointment, you feel the support worker is not the right fit, the cost of that session (up to 2 hours) will be waived—no questions asked.

   • The first appointment will be billed at your usual NDIS rate, with a maximum refund or waiver of up to 2 hours.

   • To claim the refund, clients (or their representative) must notify us in writing within 48 hours of the appointment ending. Email or SMS is acceptable.

   • If no contact is made within 48 hours, the appointment will be billed in full and the offer will no longer apply. The amount will be considered non-refundable.

3. Claim Process

       Please include your name, the worker’s name, appointment date, and a brief message indicating you would like to access the “Profile Promo” offer.

4. Limitations

   • This offer is limited to one appointment per new client.

   • The offer cannot be transferred, exchanged for cash, or applied to past or future appointments.

   • The offer only applies to support work services and does not include transport or other additional charges unless otherwise stated.

5. Right to Withdraw or Modify

   • Choice Community Health reserves the right to withdraw or amend this offer at any time without notice, but any appointments already made under the promotion will still be honoured under these Terms and Conditions.
