Depending on your eligibility, services can be funded through various sources, including: NDIS (National Disability Insurance Scheme): If you are an NDIS participant, services are billed directly to your NDIS plan under the relevant funding categories. Aged Care Packages: Clients with Home Care Packages can allocate their funding to cover eligible services. It does not cover rent, groceries, or personal expenses but focuses on providing the necessary support to help participants live comfortably and independently.
  • How Do I Choose My NDIS Support Providers?
    Choosing the right NDIS support providers is essential to getting the most out of your plan. Here are some key tips to help you decide: Understand Your Needs: Identify the services and supports you require to achieve your goals. Check Registration: Ensure the provider is registered with the NDIS if you are NDIA-managed. This guarantees they meet quality and safety standards. Ask Questions: Find out about their experience, qualifications, and the types of support they offer. Compare Options: Look at different providers to see who aligns best with your needs, values, and preferences. Read Reviews: Check feedback from other participants to understand their reputation and quality of service. Consider Flexibility: Choose providers who offer flexible services tailored to your schedule and changing needs. You have the freedom to switch providers if you’re not satisfied with their services.   • What does ‘reasonable and necessary’ mean?
    NDIA staff make decisions about what is ‘reasonable and necessary’ based on the rules within the National Disability Insurance Scheme Act 2013 (NDIS Act). To be considered reasonable and necessary, a support must: be related to the participant’s disability not include day-to-day living costs that are not related to a participant’s disability support needs represent value for money be likely to be effective and beneficial to the participant, and take into account informal supports given to participants by families, carers, networks, and the community.
  • Is the NDIS means tested?
    No, the funding under the NDIS is not means tested. Support is related to a child’s disability and the supports they need, not their parents’ capacity to pay for support.
  • Who controls my NDIS funds?
    You do! Your NDIS plan will tell you how much funding you will receive for each support but it’s your decision how funds are managed. You can choose to manage your funds yourself or nominate the NDIS, a registered provider or someone else you trust to do it for you. With the NDIS you’ll always retain control over how, when and where your supports are provided.
  • I’m over 65. Can I access the NDIS?
    No. If you’re over 65, you can’t access the NDIS – but that doesn’t mean you won’t receive support. If you’re aged 65 or over and are currently receiving disability supports, you’ll continue to receive the same level of support as before, just not through the NDIS.
  • Can I be a Choice Community Health customer if I dont have an NDIS plan?
    If you are not eligible for NDIS funding, you are still able to access our services. We do accept funding from other funding sources on a case-by-case basis. Please contact us to discuss your options.
  • Should I ask for a Support Coordinator to be included in my plan?
    That is entirely your choice. You can self-manage the coordination of your services and supports or request that an agency, such as Choice Community Health, coordinate your supports on your behalf. They will source the most suitable services and supports for your needs and connect you with providers. Your support coordinator will also help you decipher your plan and assist you with any issues and reviews. Unless you want to do your own coordination you must request this in your plan.
  • What Choice Communtiy Health Servcies can I access through the NDIS?
    All our current services for children and adults will be available to you under NDIS plus more. You can use you funding to create your own 1:1 program, services and supports and/or join our community groups and activities. We offer a wide range of NDIS specific services, including: Support Coordination: Helping you navigate your plan and connect with providers. Psychosocial Recovery Coaching: Tailored mental health support to build independence. Community Support: Assistance with daily activities, social participation, and skill-building. Therapies: Counseling and allied health services to improve well-being and functionality. Our services are designed to empower you to reach your goals while maximising the benefits of your NDIS plan.
  • What is a service agreement?
    A service agreement is a legal agreement between you and your service provider/s which outlines the support that will be provided for you under the NDIS. It describes how and when the support will be provided, as well as the costs and the fees you need to pay.
  • What is a Support Coordinator, and how can they help me?
    A Support Coordinator helps you implement your NDIS plan by: Explaining your plan and funding categories. Linking you with service providers and community supports. Assisting with budgeting and ensuring your funds are used effectively. Helping resolve issues with providers or services if they arise. At Choice Community Health, our Support Coordinators work closely with you to ensure you get the best out of your NDIS plan.
